Conversion Total Hip Arthroplasty in Bony Ankylosis of the Hip Joint

Abstract
Introduction: We analyzed the results of conversion total hip arthroplasty (THA) in bony ankylosis of hip joints.

Materials and Methods: Between August 1987 and Febuary 1998, 38 hips underwent conversion of an ankylosed hip to a THA. The average age at the time of operation and follow up period were 38 years old and 6.9 years, respectively. 27 hips had spontaneous ankylosis and 11 surgical ankylosis. Preoperatively, 15 patients complained of lower back pain, ipsilateral or contralateral knee or hip pain. 23 patients were unable to do light house work, due to severe limb discrepancy and hip deformity, prior to the opertion. We evaluated both the clinical and radiographic parameters.

Results: The aveage postoperative Harris hip score improved from 68.7 to 87. No limping gait was seen in 14 of the 38 cases following the opertion. The average ranges of hip motion were 0¢ª of flexion contracture, 87¢ª of further flexion, 27¢ª of external rotation, 19¢ª of internal rotation, 25¢ª of abduction and 23¢ª of adduction. In a radiolographic evaluation, osteolysis was seen in 6 hips and loosening in 2. The complications included proximal calcar crack, heterotopic ossification, deep infection and transient sciatic nerve palsy in 6, 10, 2 hips, respectively.

Conclusion: The conversion THA could provide pain relief, and improve leg length discrepancy and the range of motion
